At the core of the project was the design and launch of the Coordinated Entry Homeless Services System at the Santa Cruz County Human Services Department. The goal was to create a user-centric system facilitating assessment and referral for individuals experiencing homelessness. Leading the effort involved developing a project plan through extensive stakeholder engagement with community organizations. I supported the establishment of workflows enabling seamless client transition from outreach to housing stages. Additionally, a robust data infrastructure was set up to monitor referral and program outcomes, ensuring compliance with Housing and Urban Development (HUD) regulations. Staff training initiatives were implemented countywide to foster a collaborative learning environment. The outcome was the successful launch and implementation of an integrated homeless services system, benefitting thousands of individuals to date.
