Unite Us Rollout & Adoption

At the forefront of the project was the introduction of the Unite Us referral system within a prominent social services agency, aimed at revolutionizing client care coordination. This platform facilitated seamless electronic referrals among various programs, ensuring no vulnerable individual slipped through the gaps in support. My pivotal role involved devising and executing an intricate work plan, fostering relationships and workflows among a diverse array of programs across Santa Cruz County. Additionally, I led the orientation and training of staff to drive widespread adoption. My approach prioritized problem-solving and customization to meet the unique needs of each participating program, ensuring a user-centric process at every step. The culmination of these efforts saw the successful integration of the Unite Us program across 28 programs, marking significant progress in bolstering client care coordination and support services throughout the client's community.
